CASE OF THE MONTH 26 (September 2017)
34 year old male•C/o Generalized weakness 3-4 months
•Swelling over lower limbs since 3 months
•Seen by local physician: Urine albumin 3+, RBC 6-8/hpf, Creatinine 1.3 mg%
•USG- bilateral normal sized kidneys; CMD maintained
•Also revealed h/o similar complaints 8 months back: seen by a Nephrologist- investigated & renal biopsy was performed
•Reported as Focal and Segmental Glomerular Sclerosis (FSGS- NOS): slides not available for review
•Did not follow up with that Nephrologist
•Received indigenous medications with partial remission of pedal edema!!
•Now referred by local physician to a different Nephrologist

- Peripheral smear: Normocytic normochromic
- Urine: Albumin 3+, RBC 2-3/hpf, pus cells 1-2/hpf
- 24 hours urinary protein: 3200 mg
- Urea: 48 mg%
- Creatinine: 1.3 mg%
- Cholesterol: 397 mg%
- Triglycerides: 218 mg%
- Liver function tests: WNL
- ANA & anti ds DNA: Negative
- HBsAg/HCV/HIV: Non reactive
- C3 & C4 levels: 118 mg% and 26 mg% (WNL)
